About the Company:

Central Florida Communities, Inc. (CFC) has warm, welcoming, home-inspired facilities and programs for individuals with developmental disabilities in Orange, Brevard, Lake and Seminole Counties.

CFC operates three 64-bed Intermediate Care Facilities for the Developmentally Disabled (ICF/DD's), Howell Branch Court, Lake View Court and Washington Square. As a member of our staff, you will be a part of one of these living centers and provide vital around the clock care, which includes nursing and medical oversight, to our residents.

As a member of our staff, you will be providing vital administrative support to one of our many partners throughout Florida.

Purpose:

the purpose of this position is to act as contact person with local, state and federal agencies and as liaison to public and private service providers, including but not limited to Agency for Persons with Disabilities (APD), Medicaid, Medicare, Agency for Health Care Administration (AHCA), Social Security and Veteran Affairs.

Essential Duties:

Complete all initial and annual applications for applicable funding sources.

Complete the selection, purchase, maintenance and inventory of all personal effects to include clothing, shoes, personal care items, room decorations, and any other preferred items/activities as an advocate for residents’ financial interests. Adhere to all Accounting policies and procedures.

Review all resident accounts/trust accounts and coordinate management of account balances and coordinate balance management with guardian, bookkeeper and accounting. Interface with guardian, find potential guardians, facilitate the process with attorney and assist the guardian with setting up the account. Work with guardian to maintain proper balances throughout the year.

Assist residents and families to participate in the development, understanding and implementation of Support Plans. Participate in developing and implementing Human Growth and Development skills as needed with our residents.

Assist residents and families in obtaining guardianship and pre-funeral arrangements if not already in place which includes facilitation and completion of required documents for individuals involved in the process.

Required Qualification:

Bachelor in Social Work degree

Current / valid Florida Drivers license

For purposes of effective communication with residents, families and staff must be able to speak, read and write English.

Preferred Qualifications:

Graduate degree preferred; such degree(s) must be from a college or university accredited or approved by the Council on Social Work Education or comparable body.

Salary/Hourly Rate:

$30.00 Hourly
