Homeless Outreach Case ManagerPart-time (20-29 hr/wk)BENEFITS:  paid vacation, personal and sick time$18/hour

  

SCHEDULE:  Part Time, 20-29 hours per week

 

GENERAL STATEMENT OF DUTIES: Promotes permanent housing opportunities for persons that are homeless through direct support and community networking.

 

ESSENTIAL RESPONSIBILITIES:

  1. Travels to community locations where persons that are homeless congregate to ensure effective local outreach.

  2. Connect persons who are homeless to emergency shelter and/or necessary resources. 

  3. Provides transportation via bus tickets or in vehicle, as applicable.

  4. Links homeless seeking housing assistance to local coordinated entry access points.

  5. Conducts in person screening assessments to assist in housing program determinations.

  6. Assists to locate safe affordable rental housing for clients.

  7. Coordinates with housing program staff to facilitate program placement for clients.

  8. Works closely with the case management team to ensure seamless service delivery from outreach to program participation.

  9. Provides training about coordinated entry to community stakeholders.

  10. Documents coordination meetings and outreach that is provided.

  11. Actively participates in regular staff meetings for the purpose of program consistency, monitoring and effectiveness

  12. Provides crises intervention as appropriate.

  13. Participate in community-wide events promoting housing services.

  14. Complies with agency policies and procedures, COA regulations, federal and state requirements, and educational/certification/registry requirements.

  15. Other duties as assigned.

Requirements

  QUALIFICATIONS:

  1. Demonstrated organizational, interpersonal, and communication skills

  2. Willingness to learn and apply best practice models as appropriate

  3. Valid Ohio Driver's License and ability to meet agency requirements for driving insurability if applicable; reliable transportation

  4. Must be able to meet agency requirements for criminal back ground check (Ohio Bureau of Criminal Identification & Investigation). 

     

*MINIMUM EDUCATIONAL/LICENSURE REQUIREMENTS: *High School Diploma or equivalent required, college coursework preferred

 

MINIMUM EXPERIENCE REQUIREMENTS: Experience with the issues of homelessness, domestic violence, chemical dependency, mental health preferred.
