At least one year of direct Home Health Care Experience. Must be an RN/BSN. The individual will be responsible to open Home Health Care cases and manage the case until DC. Would be the Case Manger and working with a homecare multidisciplinary team in clients home. Would be responsible for an on going case load and managing client's chronic diseases and medical stability. The goal would be to manage the client at home and prevent unnecessary hopitalizations. Utilization of an EMR and Oasis. Basica computer skills required. Candidates will need to complete full physical/medical exam including drug screen. Candidates must also undergo Criminal background check, reference check, driving record check.
