POSITION SUMMARY:

To be fully engaged in Quality/No Harm, Customer Experiences, and Stewardship by performing treatment/ education and providing appropriate age-specific services under the direction of a RN.

PRIMARY ACCOUNTABILITIES:

Engagement:Associate supports engagement through adherence to the I-CARE commitments.

Quality/No Harm:

  1. Adheres to all Health First, Inc. and Health First Home Care Policies and Procedures.
  2. Provides nursing care and prescribed treatments for patients in their home, according to the physician's plan of care as supervised by RN.
  3. Reports alterations to the plan of care as indicated by changes in observed condition of patient to the appropriate RN.
  4. Assures the safety of the treatment procedures, patient, and environment at all times.
  5. Assists with the orientation process of new associates.
  6. Complies with State and Federal Regulations, JCAHO standards in reference to delivery of Home Care Services.
  7. Actively participates in Quality Improvement processes.
  8. Accepts assignment willingly regardless of geographic territory.
  9. Attends Staff Meetings - (80% Minimum).
    1. Assists in organization of patient care, scheduling and documentation reviews as requested by supervisor.
    2. Communicates to Case Manager and/or PI Supervisor any adverse findings to patient condition and documents findings in the medical record.
    3. Notifies PI Supervisor of any change in their projected scheduled visits and number of hours worked to not exceed 40 hours a week.
    4. May be scheduled to work weekends.
    5. Demonstrates effective administration of medication and IV therapy.
    6. Participates in all aspects of Telehealth.

Customer Experience:

  • Meets or exceeds Press Ganey Patient Satisfaction goals as established every year.
  • Performs other related duties as assigned or requested.
  • Informs patient/family of visit schedule in advance by calling the night before and/or if schedule changes.

Stewardship:

  • Ensures appropriate utilization of visits and services for care under the direction of the RN as noted through proper documentation completed within 2 days of the visit.
  • Utilizes supplies in a cost-effective manner.
  • Meets productivity requirements.

QUALIFICATIONS REQUIRED:

  • Graduate of an accredited school for LPN Nursing.
  • Current licensure in the State of Florida as a LPN, or endorsement
  • Minimum of 3 years acute care experience.
  • Home Care experience preferred
  • Current American Heart Association (AHA) Basic Life Support (BLS) Healthcare Provider Certification
  • Successful completion of all post-hire requirements, including, if applicable for role, Level II background screening/fingerprinting.
  • Current Florida Driver’s License, automobile insurance and reliable transportation.
  • Good communication skills.
  • Maintain all educational requirements outlined by regulatory entities and the organization
  • Proficiency in MS Office, electronic record and other systems and software relevant to role.

PHYSICAL DEMANDS:

  • Possible exposure to bio-hazardous waste, chemical substances, and unknown diseases.
  • Care is provided in a variety of home environments with variable temperatures.
  • Exposure to animals in the home.
  • Inclement weather. Move in crowded conditions.
  • Operate automobile in variable conditions
  • Visual acuity and hand-eye coordination to perform clinical tasks.
  • Lift, turn, pull, or ambulate with patients up to 20 times daily; lift up to 40 pounds unassisted up to 6 times daily.
  • Stand, walk, bend, and stoop for 90% of workday.
  • Visual acuity; minimum of 3-5 hours of computer time a day.

  • Ability to work in stressful environments/situations.
  • Ability to decide concerning appropriate patient care.
