Job Description:Smiling Angels Home Health Agency is currently seeking a compassionate and skilled LicensedPractical Nurse (LPN) to join our team in providing exceptional home health care services to ourpatients in the Louisville, KY area. The LPN will be responsible for delivering high-qualitynursing care under the supervision of a Registered Nurse or physician.Responsibilities:1. Provide direct patient care in the home setting, including performing nursing assessments,administering medications, and providing wound care.2. Collaborate with the RN and healthcare team to develop and implement individualized careplans for patients.3. Monitor and record patient vital signs and symptoms, and report any changes to thesupervising RN or physician.4. Assist patients with activities of daily living, mobility, and personal care as needed.5. Educate patients and their families on proper care techniques, medication administration, anddisease management.6. Ensure compliance with all regulatory requirements and agency policies and procedures.7. Document all patient care activities accurately and in a timely manner.
