New Vitae Wellness and Recovery Personal Care Aide in Limeport, Pennsylvania

This employer will be on-site conducting interviews at our Multi-Industry Job Fair which will be held on Thursday, May 16, 2024, at the Trolley Barn Public Market, 116 E. Provides assistance for adults who reside in a personal care home setting, most of whom are recovering from a behavioral health disability. This assistance involves a variety of direct care services to meet the physical, emotional, and social needs of assigned residents. Includes day, evening, and weekend shifts, as scheduled, and may require mandated coverage when the resident needs require.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  1. Provides one-to-one support to assigned individuals in completing Activities of Daily Living (ADL's) in accordance with their support plan; these may include assistance with bathing, toileting, dressing, and other personal care activities.
  2. Provides  one-to-one support to assigned individuals in completing Instrumental Activities of Daily Living (IADL's), in accordance with their support plan; these may include cleaning, laundry, shopping, providing transportation, support with recreational, educational or vocational pursuits, using the telephone, etc.
  3. Is familiar with the Recovery Plan and Safety Plan for each assigned individual and provides interventions accordingly.
  4. Monitors and reports on the progress of assigned individuals, as related to their support plan.
  5. Documents and communicates information regarding individuals assigned and work area through use of the Daily Communication Log, and effectively manages change of shift communication at the beginning and end of each shift.
  6. Participates as a member of multi-disciplinary team for assigned individuals, accurately and fully communicating pertinent information about their physical, emotional, or behavioral changes, assisting team with developing objectives and interventions to help individuals achieve their goals.
  7. Monitors and maintains safe and clean living environment, including replenishing of supplies and a daily quality assurance check on the rooms of all individuals assigned.
  8. Coaches, counsels, monitors and documents the diet and nutritional intake of individuals, as assigned.
  9. Completes routine checks to ensure individuals assigned are present, well, and free from possessing contraband, and that the facility is safe and secure.
  10. Completes safety checks, as assigned, for any individuals who may be at risk medically or of hurting themselves or others.
  11. Monitors interactions among individuals residing at the home, and provide support/intervention, when necessary, to ensure the physical and emotional safety of all.
  12. Reports all serious incidents, injuries, potential emergency situations, vehicular accidents, and breaches in policy and procedures to the supervisor on duty.

ADDITIONAL DUTIES AND RESPONSIBILITIES

  1. Completes and tracks a minimum of 12 hours of training annually, and all mandatory trainings assigned.
  2. Attends monthly community/staff meeting and all other meetings, as assigned.
  3. Maintains confidentiality of information related to the individuals served.
  4. Interacts in a respectful manner with individuals served, coworkers and visitors.
  5. Maintains security of keys and walkie talkies assigned during shift.
  6. Abides by all NVW&R Policies and Procedures
  7. Abides by, supports and models the NVW&R Platform Statements (Mission, Vision, Core Values, Ethical Standards, and Service Model).
  8. Abides by employee health and safety policies, including use of Standard Precautions and Infection Control procedures.
  9. Other duties, as assigned

KNOWLEDGE

  1. Working knowledge of Pennsylvania Department of Public Welfare regulations related to Adult Residential Care, as well s the department's Regulatory Compliance Guide (RCG), as related to this position.
  2. Knowledge of the Philadelphia Department of Behavioral Health Practice Guidelines, as related to this position.
  3. Certification in First Aid and CPR
  4. Fluent in English and literate.

QUALIFICATIONS

  • *Education: * High school diploma, General Equivalency Diploma, or active status on the Pennsylvania Nurse Aide Registry.

WORKING CONDITIONS

*Physical Demands: *

  1. Able to sit, stand, and walk for periods of time
  2. Able to lift up to 50 lbs.
  3. Able to hear ordinary conversation and phone communications

*Equipment Operation: *

  1. Able to operate common household equipment (e.g. vacuum, washing machine, dryer, etc.)
  2. Able to operate common office equipment.
  3. Ability to drive/transport individuals in own vehicle and/or company vehicle

*Hazards of Position: *

  1. Physical/verbal aggression of residents
  2. Exposure to household cleaning products and second-hand smoke
  3. Slight risk of contact with blood-borne pathogens
