Title: Hospice Triage RN

What You’ll Do:

• Provide support to staff and telephone interventions to maintain patients safely in their home.

• Scheduling home visits as needed.

• Collaborate with Physician practices and hospital staff to influence effective management of patients in their homes.

  What You’ll Bring:

• Current R.N. license in the State of Maine.

• Current CPR card required.

• One year of recent clinical nursing experience required.

• Hospice experience required.
