Completes patient assessments for annual wellness visits and proposes appropriate orders as needed to the primary care provider. Manages a personal provider schedule based on an identified number of visits required to meet the network’s goals. Provides education as needed to patients. Collaborates closely with physicians and APPs, practice leaders, and clinical quality teammates to ensure the patient’s plan of care follows standard work and meets quality measures. Will rotate to various clinics throughout the network for completion of annual wellness visits and to serve as a staff nurse if needed.

Qualifications include NC or Multi-State RN license.
