Helping Hands Home Care Service Social Worker in Hermitage, Pennsylvania

Helping Hands is expanding and looking for a Social Worker to be part of our company growth and diversification.  Helping Hands is looking for Full Time Social Worker for our Home Health division. BASIC PURPOSE The Social Worker assists and counsels patients and families with financial, social and emotional concerns. The Social Worker aids patients and their families with personal and environmental difficulties which interfere with obtaining maximum benefits from home health care. The Social Worker assists in coordinating discharge plans for home health patients. The Social Worker is responsible for the clinical supervision of all social work assistants providing home health care services. PRIMARY RESPONSIBILITIES   * Provides social work in accordance with the home health plan of treatment/plan of care. * Assists the physician and other members of the home health team to understand the significance of social and emotional factors related to the patient’s care. * Assesses and reassesses social and emotional factors in order to help the patient and family to cope with problems of daily living and to help them follow medical recommendations to maximize the patient’s health condition relative to their prognosis. * Recognizes and utilizes community and family resources to assist the patient in long-term plans for life in the community or to learn to live within the patient’s disability. * Counsels patients and families concerning the facilitation of the medical plan of treatment. * Within 72 hours of referral, develops the plan of care with the physician, home health team, and patient/family to deal with personal, financial and environmental difficulties that may interfere with the health and wellness continuum. * Assists and participates in discharge planning for the patient. * Develops, prepares, maintains and submits individualized patient care progress records with accuracy, timeliness and according to agency policies. * Maintains continuity of patient care with other health professionals and community resources. * Attends/coordinates patient care conferences. Initiates patient conferences of complex family and patients whose needs require measures beyond routine care conferences. * Completes a minimum of 12 hours of inservices per year. * Participate in meetings and other related activities as needed. * Assists with orientation of new personnel and education of all staff as assigned. * Understands the goals, functions objectives and operations of the agency as outlined in the policies to ensure appropriate functioning of the agency. * Utilizes supplies and personnel in a responsible, cost-effective manner to achieve patient outcomes. * Provides supervision to home health social work assistants providing services by a variety of means including but not limited to: * Supervisory visits at the patient’s home made at minimum annually with the performance evaluation but more often as indicated based on patient’s and/or the social work assistant’s need. * Review of social work clinical documentation to verify compliance with state and federal standards. * Providing direction and instruction regarding the provision of social work services. * Conferring on an as needed basis and case conferencing on individual patients either face-to-face or by telephone. * Being accessible by phone when the social work assistant is on assignment in a patient's home. * Demonstrates knowledge and ability to care for patients utilizing a developmental perspective. * Meets or exceeds delivery of service standards in a consistent fashion. * Adheres to the agency’s Code of Conduct. * Complies with local, state and federal laws, Medicare Home Health regulations and established agency policies and procedures. * The primary responsibility statements are intended to be a representative summary of the major duties and responsibilities performed by incumbents of this job. The incumbents may be requested to perform job-related tasks other than those stated in this description. * HUMAN RELATIONS   * Must maintain a professional attitude and demeanor and be able to communicate effectively and relate courteously and cooperatively with patients, caregivers, colleagues, Clinical Manager, co-workers and all others. * Must be able to demonstrate optimism, enthusiasm and willingness to work constructively with other health care team members and reflect concern for the well-being of patients and others.   * PHYSICAL REQUIREMENTS (See Attached Physical Requirements Form)   * This position requires the Social Worker to work in a variety of environments (housing styles, cleaning and maintenance, heating and cooling) during visits to patients in their place of residence.  * Visual/hearing ability must be sufficient to comprehend written/verbal communication. * The Social Worker must have ability sufficient to ambulate throughout a patient's home, navigating stairs and other physical barriers typical to private residences. * The Social Worker must be able to operate a motor vehicle and travel to and visit in patient’s homes in varying weather and road conditions. * Must be able to deal effectively with stress.   * EDUCATION AND EXPERIENCE   * Education: Bachelor's degree from a school of Social Work accredited by the Council of Social Work Education. * Experience:  Minimum one year of experience in social work in a health care setting * Licensure, Registration, and/or Certification: * Licensed as a Social Worker in Pennsylvania or meets all education and experience requirements for licensure in the state. * Valid driver’s license and reliable transportation that is insured in accordance with Pennsylvania requirements. * Satisfactory references from employers and/or professional peers. * Satisfactory criminal background check.
