This is a hospital based liaison. Clinical evaluation of patients for hospice eligibly and knowledgeable about the Medicare hospice benefit and levels of care. Fast paced work environment, hospital based, excellent verbal and written skills as well as excellent customer service skills.

Job Summary:

Build and maintain relationships with referral sources, educating patient/families on the hospice benefit and services, eligibility evaluation of patient, coordination of care with providers and hospice team, ensuring regulatory compliance.

Job Requirements:

• SC RN with license in good standing

• Strong communication skills

• Computer proficiency

• At least one year of nursing experience with strong Hospice experience preferred,

• Knowledge of hospice eligibility and criteria for levels of care preferred

Work Days:

Hours - 8:30 - 5pm. Flexibility required (based on referral volume and patient/family request for evening appointments)
