Job Summary:

As a Hospice Admission RN, you will play a pivotal role in providing compassionate and comprehensive care to patients entering our hospice program. You will be responsible for conducting thorough assessments, coordinating admissions, and facilitating the transition of patients. their families, and senior communities into our hospice services. This role requires strong clinical skills, excellent communication abilities, and a deep commitment to delivering high-quality end-of-life care.

Job Functions:

  • Conduct comprehensive assessments of patients referred for hospice services, including physical, psychological, social, and spiritual needs

  • Collaborate with physicians, other healthcare professionals, and hospice interdisciplinary team members to determine eligibility for hospice care and develop individualized care plans

  • Educate patients, families, and communities about hospice services, treatment options, and advance care planning, ensuring informed decision-making and alignment with patient preferences

  • Assist the intake team to coordinate the admission process, including obtaining necessary consents, completing documentation, and arranging for appropriate services and equipment

  • Conduct initial nursing assessments and provide skilled nursing care as needed to ensure symptom management and comfort for patients

  • Serve as an ambassador between patients, families, caregivers, communities, and the interdisciplinary team to facilitate communication and ensure continuity of care

  • Provide emotional support to patients and families coping with end-of-life issues

  • Monitor patient status, response to treatment, and changes in condition, and collaborate with the interdisciplinary team to adjust care plans as necessary

  • Document all assessments, interventions, and communication in accordance with regulatory requirements and organizational policies

  • Participate in quality improvement initiatives, clinical meetings, and continuing education to enhance skills and promote best practices in hospice care

Qualifications:

  • Current licensure as a Registered Nurse (RN) in the state of Colorado

  • Minimum of 2 years of clinical nursing experience, with preference given to candidates with experience in hospice, palliative care, or end-of-life care

  • Strong assessment skills and clinical judgment, with the ability to prioritize and manage multiple tasks effectively

  • Excellent communication and interpersonal skills, with the ability to establish rapport and provide compassionate support to patients and families along with community partners

  • Proficiency in documentation and electronic health records (EHR) systems

  • Commitment to upholding ethical standards and promoting a culture of dignity, respect, and sensitivity in end-of-life care

  • Ability to work collaboratively as part of a multidisciplinary team and adapt to changing situations in a fast-paced environment

  • Reliable transportation and willingness to travel to patient homes, hospitals, and other healthcare settings as needed

  • BLS certification required; additional certifications in hospice and palliative care preferred

The salary for this position is competitive and commensurate with experience. The pay range for this role in the state of Colorado typically falls between $55.00 - $60.00 hourly with the potential for performance-based bonuses and other benefits. Actual compensation may vary based on factors such as qualifications, experience, and location within the state.
