Job Overview

The Behavioral Health Care Coordinator (BHCC) works with multidisciplinary teams, psychosocial support providers and self-management teams, managing a specific panel of patients identified as having rising risk or high risk. Responsible for the coordination of care across the system, organizing care activities and sharing information to achieve safer and more effective care. Responsible for enhanced care coordination and care management services, coordinating, facilitating, and integrating mental health treatment, care, and support, which is tailored to meet the specific needs of patient. Ensures that the care provided is patient and family centered, evidence-based, cost-effective, holistic in nature, and leads to better outcomes. Responsible for partnering with the patient and family for seamless transitions in care between the primary care provider, treatment team, hospital, home, school, and community.

Qualifications

Education: Master's in Human Services or related field. Experience: Four (4) years in Behavioral Health field; training with an emphasis in pediatric care coordination. American Heart Association Basic Life Support for Healthcare Provider or American Red Cross CPR for the Professional Rescuer or ASHI Basic Life Support for Health Providers Licensure(s): Licensed Social Work (LSW) or Licensed Clinical Social Work (LCSW) or Marriage and Family Therapy (MFT) or Licensed Professional Counselor (LPC) within state of Colorado is required.
