Description

Klarus Home Healthis seeking aDirector of Clinical Servicesto join our team. The Director of Clinical Services is alicensed physician, physical therapist, speech-language pathologist, occupational therapist, social worker, or a registered nursewho provides oversight of all patient care services and personnel within ourAlbuquerque, NMand surrounding territories.

This role ensuresquality patient carein the home and coordinates care with the interdisciplinary team, patient, family, and referring agency.

Titles for this position may also include Director of Nurses, Supervising Nurse, Director of Clinical Services, Director of Professional Services, or similar.

Requirements

  • Education:Graduate from an accredited college/university with a degree in the discipline of the professional license held.

  • Licensure:Active state license as a physician, physical therapist, speech-language pathologist, occupational therapist, audiologist, social worker, or a registered nurse. Current driver's license.

  • Experience:One year of experience in home health care preferred. Management experience preferred.

  • Skills:Clinical skills as defined by generally accepted standards of practice. Excellent interpersonal skills and effective communication abilities. Demonstrates proven decision-making skills. If required to make visits, proof of current CPR certification.

  • Transportation:Reliable transportation and valid auto liability insurance.

Environmental and Working Conditions:

  • Performs duties in an office environment.

  • If required to make patient visits, may work in patients' homes under various conditions.

  • Possible exposure to blood, bodily fluids, and infectious diseases.

  • Ability to work a flexible schedule and travel locally.

  • Some exposure to unpleasant weather.

  • PRN emergency calls required.

  • Must be available at all times either in person or via telecommunications.

Physical and Mental Effort:

  • Prolonged sitting required, with the ability to operate office equipment, which may include heavy lifting, bending, and standing.

  • Ability to handle stressful situations in a calm and courteous manner.

  • If required to make patient visits, prolonged standing and walking required, with the ability to lift up to 50 pounds and move patients.

  • Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition.

Essential Functions:

  • Makes patient and personnel assignments.

  • Coordinates patient care.

  • Ensures communication with all physicians involved in a patient's plan of care and other healthcare practitioners.

  • Integrates orders from all relevant physicians into the plan of care and ensures the orders are approved by the responsible physician.

  • Coordinates and integrates care provided by agency staff, including contractual providers.

  • Coordinates referrals.

  • Ensures continual assessment of patient needs and reassessment when there is a significant health status change, at the physician's request, and after hospital discharge.

  • Develops, implements, and updates individualized plans of care in partnership with the patient, representative (if any), and caregiver(s).

  • Ensures services are provided as ordered by the physician, including patient and caregiver education and counseling.

  • Prepares clinical notes as appropriate.

  • Ensures continuity, quality, and safety of services delivered in compliance with state and federal regulations.

  • Participates in activities relevant to professional services, including development of qualifications and assignment of Agency personnel.

  • Participates in Agency-sponsored in-service training.

  • Recommends resources needed to provide care.

  • Collaborates on development and implementation of policies and procedures supporting care provision.

  • Collaborates with Agency leadership and staff to develop the organizational plan for patient care.

  • Monitors and participates in the QAPI Program and ensures appropriate corrective measures are performed.

  • Promotes compliance with all fiscal intermediary and/or other third-party payors through education and coaching.

  • Identifies problem areas and reports to Administrator both verbally and through written reports.

  • Meets mandatory continuing education requirements of the Agency and licensing board.

  • Promotes and educates on infection control and standard precautions to prevent contamination and disease transmission.

  • Uses effective interpersonal relations and communication skills; facilitates these skills in other team members to achieve desirable outcomes.

  • Ensures consistent availability of care offered by the Agency, including branches, to all patients.

92,000 to 100,000 (based on experience)
